One thing that might work for you is iMotion Pro on the iPad, and iMotion Remote on the iPhone (or vice-versa). This is a cool app that will allow you to shoot stop motion & time lapse videos, but I am sure you can use it to shoot single-frame shots as well.

Apart from iMotion Pro: Wifi Camera, IWatcher Pro and Remote Shutter (works through bluetooth) are the ones I found, that do this. Have fun. t

TouchCast has an External Camera app for the iPhone paired with TouchCast Studio on the iPad. It's for recording webcasts. I think they are all free ( I picked them up when they were).
For video, there's also Recolive Multicam, a beautiful thing...
