The Notes app? It's pretty great these days, but doesn't have inter-note links
Bear Note does it all though, but it's a subscription app.
Notion (not the music making app; the organizing app) has moved to free. There’s no longer payment for the service. They sent me an email a few days ago.
I respect that. It's a matter of perspective. I use cloud services because it allows me to organize between multiple devices. Ideally I'd like to set up my own local server where I can back stuff up when I want. (Raspberry Pi for example).
Of course, if the work I'm doing is super secret, I still use offline tools. I even disable auto updates on apps because I value stability over new features.
A standalone Wiki app would be the ticket. I looked into some iOS Wiki apps, but the only one that sounded like it didn't require an internet connection isn't available in the US. (Here's the article: https://www.lifehack.org/articles/technology/put-a-wiki-on-your-ipad.html)
That would be a fun iOS development project, I would think. Too bad my iOS coding skills are still a few years away from being up to that task.
I didn't know that about PHP on iOS. Very interesting! I don't think I'd want to have to run a server app just to take notes, but can think of some other uses for it.

Even though my expectations from the beginning have been that it would be a year or more before I really get anywhere, it still seems like it's taking forever just to get past the basics. Oh well, I'm not going to give up.
Respect. Let's keep pushing forward to make our ideas into a reality. I'm going through C++ tutorials because I wanna make my app multi platform. Also, because I can't afford a Mac...yet. So far, so good. I'm still reviewing basic codes. Different syntax but it's similar to C for me.

Not sure if it is exactly what you are looking for, but LiquidText https://apps.apple.com/cn/app/liquidtext/id922765270?l=en is a pretty damn cool tool for linking notes to documents, linking highlights internally, also linking documents to other documents, you can add images etc too.
And you can make projects combining several interlinked documents
Bear Notes is very good. Bear is the only subscription app I have and it's well worth $15 per year
It does not have folders but supports multi-level tags which work like folders
You might want to look at Scrivener as well. Not sure how difference it is from the desktop version though but might worth a look.

I've just checked in my favorite private, offline database app that can optionally synced with iCloud if you really want that. I have it since years and it's probably one of the few apps that is still updated regularly.
What I like is the freedom in designing the items and hierarchical structure.
It's called MyStuffPro 2 and I bet no-one has heard of it but like it very much.
You can even embed URLs to other notes inside the same database and the URL even works from Safari. You only have to add a "web address" field to your item.
Tapping on the Safari icon will jump to the linked note.
